Use of electronic cigarettes (e-cigarettes) impairs indoor air quality and increases FeNO levels of e-cigarette consumers. E-cigarette manufacturers are using marketing tactics used by the tobacco industry to lure kids to conventional cigarettes. Their tactics include slick magazine ads, sponsorship of concerts and auto races, celebrity endorsements and sweet, colorful flavors.21 In addition, e-cigarettes are often aggressively placed in convenience stores near candy.
So when it comes to flavored e-cigarettes, we have this built-in concern about the popularity of those products with kids. But Congress did include mandatory considerations in the Family Smoking Prevention and Tobacco Control Act. And so, by law, when we get one of these applications, we have to consider the product’s impact on existing users of cigarettes and other tobacco products and on nonusers of the product.
Vapes can also be used with marijuana or tetrahydrocannabinol (THC). New ‘pod-mod’ style vape products are similar in shape and size to JUUL® and other vape sticks but are disposable. It is generally accepted that e-cigarettes produce fewer of the toxins found in cigarette smoke. However, it is becoming increasingly apparent that this frame is not adequate to tell the entire story regarding individual health impacts.
Interestingly, most of these reports linking COVID-19 harmful effects with smoking or vaping, are based on their capability of increasing the expression of angiotensin-converting enzyme 2 (ACE2) in the lung. It is well known that ACE2 is the gate for SARS-CoV-2 entrance to the airways [106] and it is mainly expressed in type 2 alveolar epithelial cells and alveolar macrophages [107]. To date, most of the studies in this field indicate that current smokers have higher expression of ACE2 in the airways (reviewed by [108]) than healthy non-smokers [109, 110]. The range of e-liquid flavours available to consumers is extensive and is used to attract both current smokers and new e-cigarette users, which is a growing public health concern [6]. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76].
“We’re at risk of dealing with nicotine addiction, several health concerns and potential lung diseases that we hadn’t really seen previously or at this rate from a particular product,” Dr. Joshua Mansour, a Los Angeles-based oncologist, told Drugwatch. The U.S. Preventive Services Task Force, a group of health experts that makes recommendations about preventive health care, has concluded that evidence is insufficient to recommend e-cigarettes for smoking cessation in adults, including pregnant women. Participants were first recruited in May 2021, and data analysis was conducted in December 2022. But it is not responsible for the harmful effects of smoking, and nicotine does not cause cancer. People have safely used nicotine replacement therapy (NRT) to stop smoking for many years. Nicotine replacement therapy is prescribed by doctors or is available from pharmacies. They usually contain nicotine, which is the addictive chemical in cigarettes.

The Sonoma County Board of Supervisors today voted to ban the sale of flavored tobacco products and e-cigarettes in unincorporated areas of the county. The changes to the county’s tobacco retail ordinance are in response to public health data that show e-cigarettes and flavored tobacco use increasing among high school-aged youth. Yet, they contain many substances beyond nicotine, including propylene glycol, glycerin, flavorings and potentially harmful chemicals such as formaldehyde and metals, which could pose significant health risks such as respiratory disease, cardiovascular disease and cancer. Vaping also is strongly linked with a serious medical condition that damages the lungs due to the vitamin E acetate, an additive used in tetrahydrocannabinol-containing e-cigarettes.
The city of Milwaukee last year changed its zoning regulations to prevent e-cigarette or tobacco retailers from opening within 500 feet of similar businesses or 1,000 feet of schools. The city of Monona has adopted a six-month ban on new vape shops, after three shops opened in recent years and the city rejected a fourth following the police chief’s concerns about its proximity to schools. Nguyen Anh Tri, former head of the National Institute of Hematology and Blood Transfusion, also agrees that e-cigarettes are an addictive substance and carcinogenic. Earlier last month, Minister of Health Dao Hong Lan said the ministry’s consistent stance is to ban e-cigarettes. That compares to adult rates for vaping, which at 6% in 2021 was little changed from the 7% reported in 2015. But the use of smokeless tobacco of all types remained constant statewide at 6% over the past 10 years.
On this webpage, these products are collectively referred to as e-cigarettes. The long-term health effects of e-cigarettes are not well understood yet. But the science clearly indicates vaping is not a safe or healthy alternative to smoking. New York State has enacted strong policies to protect youth from accessing tobacco products and denormalize tobacco and nicotine use to prevent lifelong addiction, illness, and early death. Validated 7-day point prevalence abstinence rates were affected less, and effect sizes for all outcomes were close to those predicted, but the lower quit rates have reduced the statistical power for the primary outcome. Another key limitation of open-label trials is that participants’ expectations can affect outcomes. In smoking cessation studies, the results can be biased if participants randomized to what they perceive as an inferior option are less likely to use their treatment or are more likely to drop out of the trial.
That figure is controversial and might be a little high, says Kenneth Warner, a tobacco policy researcher at the University of Michigan. But, he adds, “The worst critics of e-cigarettes would probably argue they’re a half to two-thirds less dangerous. But from a practical view, they’re probably on the order of 80% to 85% less dangerous, at least.” Public health experts and tobacco researchers are trying to find out. Young people who vape may be more likely to become addicted than adults.

This means that e-cigarettes (vapes) cannot be used in areas where smoking is banned, cannot be displayed at retailer outlets, and cannot be sold to people under 18 years of age, as well as being subject to other restrictions. The FDA’s tobacco center was created by Congress in 2009 and granted sweeping powers to remake the industry, including banning harmful ingredients from traditional products and authorizing new, less-harmful alternatives. Not everyone is aware that there is nicotine in most e-cigarettes in varying amounts.

The health protection agency reports that 2.8 percent of American adults used e-cigarettes in 2017. Among adult e-cig users, 58.8 percent also continued to smoke tobacco cigarettes while 29.8 percent were former tobacco smokers. Another 11.4 percent of e-cig users had never been regular tobacco smokers. As of December 2022, Juul faces 4,908 e-cigarette lawsuits combined in a mass litigation in a San Francisco federal court.
Law enforcement officers need to understand the value of collecting e-cigarette and vaping paraphernalia as evidence. Varenicline, perhaps better known by the brand name Chantix, is FDA-approved to help adults to stop smoking traditional cigarettes. But, despite the growing numbers of people who use e-cigarettes, there are no approved medication options to help them to stop using e-cigarettes. It made sense – Altria, the bad actor formerly known as Philip Morris, maker of Marlboro cigarettes, owned a 35% stake in JUUL. With so many adults kicking the cigarette smoking habit, Altria was looking for ways to sustain its financial stability. A French study from the same year found similar results, but also it found that e-cig use among smokers increased the risk of relapse into tobacco use.

Endothelial cell migration is an important mechanism of vascular repair than can be disrupted in smokers due to endothelial dysfunction [25, 26]. The decision to pursue a smoking cessation objective, even in such a controlled form, should be made only after considering national circumstances, along with the risk of uptake and after exhausting other proven cessation strategies. While the Family Smoking Prevention and Tobacco Control Act of 2009 prohibited the sale of most flavored cigarettes, the flavor ban did not extend to menthol flavored cigarettes or to any flavored non-cigarette tobacco products such as cigars ignite 600 puffs, hookahs, and e-cigarettes.
In addition, young adults were significantly more likely to use both cigarettes and e-cigarettes than adults 45 and older. Young adults with family incomes of less than 200% of the federal poverty level also were more likely to use e-cigarettes than those with greater family income. In 2019, the MCIAA was amended to expand the definition of smoking to include electronic delivery devices, also known as e-cigarettes or vapes. The amendment protects employees and the general public from hazards to secondhand smoke and involuntary exposure to aerosol or vapor from e-cigarettes.

FDA’s award-winning public education campaign, “The Real Cost,” continues to prevent youth from tobacco initiation and use. In 2017, the campaign began prioritizing e-cigarette prevention messaging to combat increasing youth vaping rates. “The Real Cost” campaign also educates teens on the health consequences of smoking cigarettes. Vaping involves using a device known as an e-cigarette—also called a vape pen, mod pod ignite nicotina, or tank—to heat up a small amount of liquid, turning it into a vapor that can be inhaled. The study showed that the e-liquids of certain cig-a-like brands contain high levels of nickel and chromium, which may come from the nichrome heating coils of the vaporizing device.
But levels are usually low and generally far lower than in tobacco cigarettes. Because most tobacco use starts during adolescence, actions to protect our nation’s young people from a lifetime of nicotine addiction are critical. Previous trials that compared ECs and NRT mostly complemented these treatments with intensive behavioral support.5 This raises an important question of whether ECs are effective without such clinical involvement. The present trial suggests that they are, but it does not provide a definitive answer. Although only minimal behavioral support was included, smokers were still asked to set up a TQD and their smoking status was checked monthly, features that are not available to smokers using ECs on their own. To see whether public health messages on EC use for smoking cessation need to include advice to use any additional support, further studies are needed that compare effects of different levels of behavioral support added to ECs. Electronic cigarettes (also known as “e-cigs,” “vapes,” and “electronic nicotine delivery systems (ENDS)”) are battery-powered devices that heat liquid into a vapor for the user to inhale.
When youth and young adults expose their brains to nicotine, they are vulnerable to nicotine addiction, may have trouble paying attention and concentrating, and experience mood disorders and reduced impulse control. Nicotine may alter the way their brains function for the rest of their lives. E-cigarette marketing, including product design and packaging, appeals to a young audience. For example, many e-cigarettes feature bright colors and fruit, candy, alcohol or other flavors that youth find attractive and interesting.
A 2021 review found people who used e-cigarettes to quit smoking, as well as having expert face-to-face support, can be up to twice as likely to succeed as people who used other nicotine replacement products, such as patches or gum. E-cigarettes are devices that heat a liquid, called e-liquid, into an aerosol that the user can inhale. Although e-liquids do not contain tobacco, they contain chemicals, usually including flavorings, and often contain nicotine, which is addictive. There is an urgent, overdue need for standardized processes for the disposal of e-cigarette devices, refills and e-liquids. Waste management and hazardous waste disposal plants are not currently equipped to handle e-cigarette waste and federal regulations still have not caught up to the need for guidance on disposal of these types of small electronics containing toxic waste. Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. E-cigarettes not only pose substantial health risks to youth and young adults, they pose a significant environmental threat (see the Truth Initiative fact sheet on Tobacco and the Environment).
Then there’s tobacco-flavored e-cigarettes, and in that case the benefit–harm equation is different, because tobacco-flavored e-cigarettes are not nearly as popular with kids as flavored e-cigarettes. And so that’s how we have been able to authorize some tobacco-flavored e-cigarettes for marketing. This is because under federal law, although the FDA has the power to regulate the tobacco industry, they can do so with certain limitations. The Department has sent a statewide health alert to health care providers and local health departments following recent reports of severe lung disease in people who have used vaping products. The cytotoxic and pro-inflammatory effects of different e-cigarette flavouring chemicals were also tested on two human monocytic cell lines—mono mac 6 (MM6) and U937 [86]. Among the flavouring chemicals tested, CAD was found to be the most toxic and O-vanillin and pentanedione also showed significant cytotoxicity; by contrast, acetoin, diacetyl freemax maxus 2, maltol, and coumarin did not show any toxicity at the concentrations assayed (10–1000 µM). Of interest, a higher toxicity was evident when combinations of different flavours or mixed equal proportions of e-liquids from 10 differently flavoured e-liquids were tested, suggesting that vaping a single flavour is less toxic than inhaling mixed flavours [86]. Also, all the tested flavours produced significant levels of ROS in a cell-free ROS production assay.
A 2018 Truth Initiative survey found that mint was among the top three favorite flavors among young JUUL users aged 12-24, meaning they chose it last time they vaped. New research shows that mint and menthol e-cigarette use among high school users rose from 16% in 2016 to 57.3% in 2019. Among high school JUUL users, 67.5% reported that their preferred flavor was mint or menthol. Another study, conducted before JUUL pulled its other flavors from the market, found that mint was one of the most popular flavors among high school student JUUL users, but that menthol was less so.
For example, tobacco-flavored e-cigarette cartridges sales dropped from 47.2% of those sold or given away in 2015 to 21% in 2018, while the sale of “other” flavored cartridges tripled in that time, increasing from 13.8% in 2015 to 42.1% in 2018. This was significant because Chantix, at one point, had a “black box warning” after reports linking the drug to psychiatric side effects. One of the most popular e-cigarettes is Juul, but hundreds of different e-cigarette brands are currently available to consumers. In June 2022, the FDA banned Juul from selling it’s products after finding the company provided “insufficient and conflicting data” on risks. In light of this spike in youth e-cigarette use, along with the recent U.S. outbreak linked to more than 2,000 lung illnesses and over 40 deaths, the AMA has called for a total ban on all e-cigarette and vaping products that do not meet FDA approval as cessation tools.
The Federal Trade Commission’s first-ever report on e-cigarette products paints a disturbing picture of surging e-cigarette sales and advertising that are likely to damage the health of America’s youth. “People can get to very high levels of nicotine exposure with these e-cigarette products, and they can use them near constantly throughout the day. So, the question we all have is, ‘Can any pharmacotherapy stand up to this challenge? Talk to your patients, including youth and young adults, about the dangers of tobacco use. Screen all patients, encourage them to quit, and refer patients to evidence-based services for help quitting.
Among those the team could identify were several potentially harmful substances, including caffeine ignite 1500 tobacco, three chemicals never previously found in e-cigarettes, a pesticide and two flavorings linked with possible toxic effects and respiratory irritation.
