But is smoking e-cigarettes (also called vaping) better for you than using tobacco products? If they are unable to quit e-cigarettes on their own, they should seek help from a health care professional or quitline. Individuals who are not yet able to stop using e-cigarettes should be strongly discouraged from simultaneous, or “dual,” use of any combustible tobacco products, including cigarettes. Continuing to smoke exposes the individual to enormous harms, irrespective of whether the individual is using e-cigarettes part of the time.
According to the CDC, 15% of EVALI patients were less than 18 years old. Brain development begins during the growth of the fetus in the womb and continues through childhood and to about age 25. Nicotine exposure during adolescence and young adulthood can cause addiction and harm the developing brain. Equally disturbing are two reported deaths and thousands who have been injured or burned because of “vape pen” explosions.
That figure is controversial and might be a little high, says Kenneth Warner, a tobacco policy researcher at the University of Michigan. But, he adds, “The worst critics of e-cigarettes would probably argue they’re a half to two-thirds less dangerous. But from a practical view, they’re probably on the order of 80% to 85% less dangerous, at least.” Public health experts and tobacco researchers are trying to find out. It now appears that e-cigarettes may present their own unique health risks, including to the respiratory and cardiovascular systems. And a lot’s being said about vaping and its effects – but there’s also some confusing or conflicting info out there. As of April 1, 2024, 1,061 municipalities, plus 26 states, commonwealths, and territories restrict e-cigarette use in 100% smokefree venues. In 2018, the rapid rise of tobacco products led the U.S. surgeon general to issue an advisory about the youth e-cigarette “epidemic.” In 2019, youth e-cigarette use increased to even higher levels. While the most recent studies show a decrease in youth use in 2020, the rate is still alarmingly high with nearly one in five high school students reporting current e-cigarette use. In May 2021, the China Health Commission (equivalent to Ministry of Health) published China’s Report on the Health Hazards of Smoking 2020,26 which concluded that EC use is unsafe.
According to the 2017 NH Youth Risk Behavior Survey, e-cigarette among youth is double that of combustible cigarettes and more than a third of 12th grade males report currently using e-cigarettes. During this time, the brain is constantly rewiring itself, which makes it far more vulnerable to nicotine addiction. This is part of why teens who vape are nearly three times more likely to begin smoking cigarettes. While secondhand smoke from cigarettes causes serious harm to others, there is no evidence so far that vaping is harmful to people around you and any risks are likely to be very low. Switching to vaping significantly reduces your exposure to toxins that can cause cancer, lung disease, and diseases of the heart and circulation like heart attack and stroke. These diseases are not caused by nicotine, which is relatively harmless to health.

If you’re pregnant, licensed NRT products such as patches and gum are the recommended option to help you stop smoking. Nicotine replacement therapy (NRT) has been widely used for many years to help people stop smoking and is a safe treatment.
E-cigarettes usually contain nicotine and may have other harmful substances too. When the liquid is used up or the battery dies, the user throws the device away. Others can be refilled with liquid, recharged, and used over and over. If you prefer to stop vaping in one step, you can ask your pharmacist or stop smoking adviser about switching to a suitable nicotine replacement therapy (NRT) product. This is an alternative way of cutting down your nicotine use until you are ready to stop completely. They also cause other serious illnesses, including lung disease, heart disease and stroke.
All this evidence would suggest that e-cigarettes are potentially less harmful than conventional cigarettes (Fig. 2) [11, 14, 22, 24, 27,28,29]. Among the 17 samples tested in this latter study 14 were identified to be counterfeit or suspected counterfeit. A third study detected nicotine in 7 of 10 nicotine-free refills, although the concentrations were lower than those identified in the previous analyses (0.1–15 µg/mL) [5].

At minimum, this outbreak dramatically demonstrates the dangers of an unregulated market in inhaled substances with no premarket review for consumer safety. E-cigarettes are hooking a new generation on nicotine – putting millions of kids at risk and threatening decades of progress in reducing youth tobacco use. It’s a nationwide crisis of youth addiction, fueled by thousands of kid-friendly flavors and massive doses of nicotine.
What we do know is that they are not free of toxins and still deliver harmful chemicals. The leading reason for e-cigarette use among adults (25 and older) was the belief that e-cigarettes may be less harmful than combustible cigarettes both to themselves and others, with 79% selecting this as a reason. Additionally, 7 in 10 (71.5%) adults indicated that they believed e-cigarettes could facilitate quitting. This is Quitting, Truth Initiative’s first-of-its-kind, free and anonymous text message quit vaping program for teens and young adults, is helping more than 200,000 young people quit using e-cigarettes. The program first launched in January 2019, created with input from teens, college students and young adults who have attempted to, or successfully, quit e-cigarettes. For use frequency pre-ban, respondents reported whether they used e-cigarettes daily (every day or most days in a week ) or weekly (at least once a week, but not most days). For both pre- and post-ban, respondents reported all e-cigarette flavors they used, the flavor that they primarily used, and the different ways they obtained e-cigarettes. Most respondents continued to use e-cigarettes with banned flavors post-ban. Compliance of local retailers with the ban was not high, and many respondents obtained banned-flavor e-cigarettes through legal channels. However, the significant increase in the use of non-flavored e-cigarettes post-ban suggests that these may serve as a viable alternative among those who used previously used banned or tobacco flavors.
District Court for the District of Columbia ruled that e-cigarettes did not meet the criteria for drug-delivery devices and therefore were exempt from regulation under the FFDCA. The court did rule, however, that the FDA could regulate e-cigarettes as tobacco products under the Family Smoking Prevention and Tobacco Control Act (TCA), since the nicotine contained in some of the e-cigarette cartridges was derived from tobacco. Reports in 2018 of increased e-cigarette use among adolescents and teenagers in the United States prompted the FDA to identify strategies for combating e-cigarette use by minors.

There are currently 78 licensed tobacco retailers in unincorporated Sonoma County, representing over 20% of the total tobacco retailers throughout the county, including cities, the county said. Given these alarming trends, the researchers advocate for targeted interventions to address the rising vaping rates among adolescents. They suggest mass media campaigns and peer interventions to counteract the social norms that promote such risky behaviors. By 2019, this figure had jumped to 7.2% – more than a threefold increase.
At baseline, demographic and smoking history variables were collected, including age, sex, ethnicity, education, marital status, income, health status, age of starting to smoke, cigarettes smoked per day, and previous cessation attempts. The retailers sold or distributed e-cigarette products in the U.S. that were not authorized by the FDA, putting them in violation of the Federal Food, Drug, and Cosmetic Act, the agency indicates. However, varenicline is already on the market in the U.S. in generic versions while cytisinicline has not yet received FDA approval and is not currently available for use by patients. To date, there is no state excise or special tax placed on e-cigarettes. These products, also known as e-cigarettes, don’t create smoke—because they don’t burn tobacco. Well, if they have, it’s not public because that’s not a public process. But one can look around and see that no e-cigarettes have been approved for use as safe and effective cessation aids. Ultimately, the legal burden is on the companies to demonstrate that the benefit to adults is going to outweigh the harm to kids.
