Nationally, e-cigarette use among high school students doubled from 11.7% in 2017 to 27.5% in 2019. Unlike cigarette butts, e-cigarette waste won’t biodegrade even under severe conditions. FDA’s award-winning public education campaign, “The Real Cost,” continues to prevent youth from tobacco initiation and use. In 2017, the campaign began prioritizing e-cigarette prevention messaging to combat increasing youth vaping rates. “The Real Cost” campaign also educates teens on the health consequences of smoking cigarettes. Vaping involves using a device known as an e-cigarette—also called a vape pen, mod, or tank—to heat up a small amount of liquid, turning it into a vapor that can be inhaled. Most vape liquids contain substances such as propylene glycol and glycerol as base ingredients that create the vapor. But they may also contain other compounds including artificial flavors, nicotine, THC, and cannabinoid (CBD) oil.

The devices, also known as vapes, can cause serious health implications, including contributing to detrimental inhalation patterns, and heightened risks for youth’s developing brains, according to medical researchers. While data indicate a substantial decline in traditional cigarette smoking among U.S. adolescents, the introduction of EVP use and their alarming increases have presented new challenges. The researchers believe that the data create clinical and public health challenges. As of August 1, 2019, this definition includes carrying or using an activated electronic delivery device, such as e-cigarettes or vapes.

With what’s known about it right now, though, experts say it isn’t safe—or even safer— to vape marijuana. Today, experts admit they still don’t fully know what e-cigarette vapor actually contains. Similarly, the proportion of individuals reporting never using combustible cigarettes among those who reported daily e-cigarette use was highest among young adults aged 18 to 20 years (66.5% [95% CI, 61.2%-71.4%]) (Table 4).

Patients have reported high variability in substances/products they used in vaping, including both tobacco and tetrahydrocannabinol (THC) containing products as well as other products. Many e-cigarettes and vaping materials contain harmful or potentially harmful substances, and it is difficult to know what each individual product contains. The mislabelling of nicotine content in e-liquids has been previously addressed [8, 34]. Of note, several studies have detected nicotine in those e-liquids labelled as nicotine-free [5, 35, 36]. Among the 17 samples tested in this latter study 14 were identified to be counterfeit or suspected counterfeit. A third study detected nicotine in 7 of 10 nicotine-free refills, although the concentrations were lower than those identified in the previous analyses (0.1–15 µg/mL) [5].

They can also be collectively called electronic nicotine delivery systems (ENDS). A non-smoker who uses ENDS may become addicted to nicotine and find it difficult to stop using ENDS or become addicted to conventional tobacco products. Further, many e-cigarettes are designed to allow the control of nicotine (which can reach dangerously high levels). This can also increase the levels of other toxicants to the user and also expose bystanders to harmful emissions.

Small and sleek, these products can look a lot like a computer flash drive or pen. This makes it tricky for adults to recognize them right away as vaping tools. Pod-based e-cigarette devices are easy to hide in a fist or a pocket. The CDC’s Office on Smoking and Health (OSH) developed this presentation to educate youth on e-cigarettes, including the health risks, the factors that lead to e-cigarette use, and what youth can do to avoid all tobacco products, including e-cigarettes.

They may want to use caution, however, if their strategy involves switching from cigarettes to e-cigarettes, considered by some to be a less harmful alternative. Moreover, while the basic technology behind e-cigarettes is consistent, there is enormous variability within the product category and there is no typical e-cigarette. The products include different ingredients, different hardware and deliver highly variable amounts of nicotine and potentially toxic chemicals, including heavy metals such as cadmium, lead, nickel, tin and copper.

This does not necessarily mean, however, that e-cigarettes are free from hazardous effects. Indeed, studies investigating their long-term effects on human health are urgently required. In this regard, the main additional studies needed in this field are summarized in Table 3. Other important components in the aerosols include silicate particles from the fiberglass wicks or silicone [89,90,91]. Many of these products are known to cause abnormalities in respiratory function and respiratory diseases [89,90,91], but more in-depth studies are required. Interestingly, the battery output voltage also seems to have an impact on the cytotoxicity of the aerosol vapours, with e-liquids from a higher battery output voltage showing more toxicity to A549 cells [30].

EVALI is a serious medical condition in which a person’s lungs become damaged from substances contained in e-cigarettes and vaping products. E-cigarettes invite harmful and unknown chemicals into the body and bloodstream. These cancer-causing agents reach deep into the lungs, irritating the bronchi and cause an increase in blood pressure, heart rate, as well as compromise adequate flow of blood to the heart and restrict of the arteries. Juul is probably the most popular brand of e-cigarette currently marketed. The device heats a nicotine-containing liquid pod to produce an aerosol inhaled by the user and any bystander within its radius (in the form of secondhand “smoke”). There may be added dangers, for example if a vape battery catches fire near an oxygen tank, a propane tank (such as used in backyard grills), or a gas pump, or if a person has a vape-related seizure while driving. “Yale physicians have been at the forefront in identifying EVALI cases,” Dr. Weiss says. She explains that even before CDC guidelines on treating the illness were released, Yale doctors were successfully treating patients with a combination of antibiotics and steroids. At baseline, demographic and smoking history variables were collected, including age, sex, ethnicity, education, marital status, income, health status, age of starting to smoke, cigarettes smoked per day, and previous cessation attempts. However, the portion primarily using non-banned, tobacco-flavored e-cigarettes also decreased (from 20.1% to 15.6%). Young adults with family incomes of less than 200% of the federal poverty level also were more likely to use e-cigarettes than those with greater family income. E-cigarette devices can be used to deliver marijuana and other drugs. Results from in vitro studies are in general agreement with the limited number of in vivo studies. For example, in an analysis using primary human umbilical vein endothelial cells (HUVEC) exposed to 11 commercially-available vapours, 5 were found to be acutely cytotoxic, and only 3 of those contained nicotine [24].
